IF YOU'RE ON TIME THEN WE'RE ON TIME

Being healthy shouldn't make your life more difficult. Instead, it should fit your schedule. At Leaf Medical, we ask our patients to respect time and arrive 10 minutes earlier than their scheduled appointment. As long as you are on time for your appointment, we at Leaf Medical, respect your time by promising little-to-no wait time. It's important that you come on time so that we can keep our promise!

At Leaf Medical, we practice the highest level of preventative medicine. In order to provide you with the best care that you deserve, we limit the number of patients to physicians enrolled in our practice by 40% less than the national average. This enables us to spend more time with you. We learn your needs, teach you about your health and help you come up with a personalized plan.

YOUR HEALTH PROMISE

As a Leaf member, we ask you to come in once a year for your annual physical. This allows us to track your health, prevent problems from happening, establish healthy habits early on and keep the number of patients low so we can spend the time needed to give you individualized care. By doing so, we waive your membership fee.

Lateness Policy

Patients are expected to arrive at least 5-10 minutes earlier than your scheduled appointment. If you are late by more than 10 minutes of your scheduled time, we will try to accommodate you by either rescheduling you for an appointment on the same day or asking you to wait for an opening near the time of your scheduled appointment time. If no accommodations can be made for the same day, you will be charged the cancellation fee of $50.00. This cancellation fee is collected because the physician was placed on standby and a patient could not be seen during that time slot, as it was reserved for you. Please respect our physicians' time and other patients' time by arriving on time for your appointments.

Cancellation Policy

If you cancel 24 hours prior to your appointment, there is no penalty as long as you reschedule. If you cancel less than 24 hours prior to your appointment, there is a cancellation fee of $50.00. This cancellation fee is collected because the physician's time slot was taken by you, and less than 24 hours may not be enough time to fill that slot with another patient.

No Show Policy

If you have an appointment scheduled and do not appear for the appointment at all, this is called a No Show. No shows will be charged a $100.00 fee. This fee is charged because the physician was on standby waiting for you. During this time slot, another patient could not be placed in that time slot because you were expected to arrive. After 3 No Shows, you will be asked to leave the practice. Please respect our physicians' time and other patients' time by at least notifying the practice 24 hours prior to your appointment.

Rescheduling Policy

You may reschedule your appointment up to 24 hours prior to your scheduled appointment without penalty. If you must reschedule less than 24 hours prior to your scheduled appointment, you will be charged the cancellation fee of $50.00. This cancellation fee is collected because the physician's time slot was taken by you, and less than 24 hours may not be enough time to fill that slot with another patient.

Missed Annual Visit Policy

By joining Leaf Medical, you agree to make your required annual physical yearly. This way our doctors can track your health progress, and the practice can manage the number of patients to doctor ratio to avoid overcrowding. If you are unable to make your mandatory annual visit despite being notified by our support staff, your membership will be automatically cancelled at no cost. However, if you choose to return for any type of visit, there is a mandatory membership renewal fee of $150, which must be paid before your next visit. Simply put, as long as you have your annual physical at any Leaf Medical location, there is no extra fee. Easy!

Q. What happens if I had my annual physical at Leaf Medical in 2018 as well as in 2019?

A. Nothing, we hope to see you back in 2020. Your membership would be in good standing, and there are no extra costs.

Q. What happens if I had my annual physical at Leaf Medical in 2017, but skipped the entire year of 2018?

A. There is no charge, and your membership was automatically cancelled in 2018. However, if you’re looking to make an appointment in 2019 at any Leaf Medical location, a charge of $150 is required to restore your membership for that year.
